Outpatient Senior Services at Hartford Hospital 

 

Hartford Hospital Senior Services provides office-based primary care and consultation for adults over the age of 60.   Our Geriatric Evaluation (Assessment) and Management Centers in three locations offer convenient access to patient and family centered care based on each individual's functional and psychosocial situation. With a staff of highly skilled experts, Hartford Hospital's Outpatient Senior Health Services address the complex needs of elderly patients with specific attention to dementia, memory impairment, frailty, and fall injuries. Our interdisciplinary team, which includes specialists in geriatric medicine, geriatric psychiatry, rehabilitation medicine, geriatric nursing, and social work, develops a comprehensive, individualized care plan for each patient.
